The regular office hours are Monday - Friday 7:30 to 5:00. We will make every effort
to be on time for your appointment and past experience indicate that your examination
will begin within 10 minutes of that time. Unfortunately, because of the emergency
nature of certain problems, occasional delays are unavoidable. In these cases, you
will be notified upon your arrival and every effort will be made to accommodate
your schedule. If you are delayed or unable to keep your appointment, please notify
us as soon as possible.
Please plan to stay with us approximately two hours to allow us to perform a complete
examination and assessment. To insure that we have an understanding of any medical
problems you may have, we will ask you to complete a patient questionnaire. You
will be examined by the doctor after preliminary testing has been done by our staff
members. During this examination your eyes will be dilated and you should make arrangements
for someone to drive you home. Based on the results of the examination and your
medical history, the doctor will develop a treatment plan for you. In some cases,
this will consist of medical treatment such as eye drops oral medications and/or
corrective lenses. In other cases, surgery or laser treatment may be required.
If you are interested in Laser Vision Correction you will be advised about its applicability
in your case. Our Doctors are skilled and experienced in this modality of treatment.
The Ocular Surgery Center, Inc. is a Medicare certified facility and the physicians
are participants in the Medicare assignments. We will file all charges with your
primary and secondary insurance carriers. We will cooperate in any way with you
and your insurance carrier in this regard. Unless otherwise agreed upon, you are
responsible for payment of any outstanding bills beyond 60 days. Should subsequent
insurance payment create an overpayment, it will be promptly refunded to you. We
do not want to be a financial burden to any of our patients. If the charges incurred
result in such a burden, please discuss this with the bookkeeping department so
that special arrangements can be made. We will work with you so as to assure that
you receive excellent medical care.
If transportation to and from the clinic for surgery presents a major problem, special
transportation arrangements can be made. Please bring this to the attention of the
appointment secretary when you are making your appointment.
Your medical record is strictly private. No information regarding your condition
will be given to your employer, friends or relatives without your permission. The
only exception will occur when disclosures are required by law in such cases as
industrial accidents, injuries, assaults, etc. We will provide your referring doctor
with a full report on your eye problems and recommended treatment.
We believe the doctor/patient relationship is best when based on mutual understanding.
We make a special effort to explain everything to you about your condition, medicines
and treatment. If you ever have questions, or if anything is not clear to you, do
not hesitate to ask about it. Our staff, office procedures and medical equipment
have been chosen with thought and care to provide quality medical services in a

This office is fully equipped for retinal referral consultations, follow-up evaluations,
and retinal laser treatment.
